What should I send for a custom flange?

A drawing with inside and outside diameters, thickness, hole details, material and quantity. Include any specification required by the system where it will be used.

Are all plate holes made using one process?

No. Material, thickness, hole diameter and geometry affect the choice. A part may combine cutting and drilling.
